In the Apply Lighting dialog box to create a new library click on the Library button, the Open Lighting Library File dialog (12) will appear.  Click on the New button and the Create File dialog (13) appears.  Enter a name and click Save.  The Apply Lighting Library window will appear blank until you save a light to it.
